Neurological Conditions

Vertigo

  • Must be symptom-free for 2 months
  • Must bring report from neurologist every 2 years if vertigo is a chronic condition.
  • Cannot be taking benzodiazipenes or phenothiazines for vertigo.


Stroke

  • If stroke occurred in brainstem/cerebellum: must present written report from neurologist annually and must be at least 1 year since the stroke occurred. Cannot have cognitive deficits or neuromuscular deficits that would interfere with driving
  • If stroke occurred in the cortical/subcortical region of the brain: must present written report from neurologist annually and must be at least 5 years since the stroke occurred. Cannot have cognitive deficits or neuromuscular deficits that would interfere with driving


TIA

  • 1 year waiting period before recertification is possible
  • Must present annual written report and clearance from a neurologist who understands the functions and demands of commercial driving


Intracerebral and Subarachnoid Hemorrhages

  • Must present annual written report from neurologist.
  • Must be at least 1 year after the event (5 years if there is any risk for seizures). If AVM, must have surgical repair documented by neurologist


Migraines, Cluster Headaches, Post-Concussive Headaches, Cranial Neuralgia, Atypical Facial Pain

  • Must present written report from neurologist, including diagnosis and treatment, if any neurologic symptoms or severe migraines.
  • If the driver has a diagnosis of post-concussive syndrome, s/he will need results from neuropsychological testing


Seizures
Single Unprovoked Seizure:  

  • Must be seizure free without any seizures medications for at least 5 years
  • Must present written report from neurologist confirming that with clearance


Two or More Unprovoked Seizures:

  • Must be seizure free without any seizure medications for at least 10 years
  • Must present written report from neurologist confirming that with clearance


Seizure secondary to Head injury:

  • Must be seizure free for 5 years and no seizure medications for at least 1 year
  • Must present documentation of neuropsychological testing and annual clearance by a neurologist


Viral/Bacterial Encephalitis or Aseptic Meningitis

  • Must present written report from neurologist stating diagnosis, no cognitive deficits and must be at least 1 year from onset (if no early seizures) or 5 years from onset (if any early seizures)


Parkinson’s Disease

  • Disqualified but may apply for exemption if stable on medications and does not experience any on/off phenomena and no cognitive deficits. Will need to apply for exemption and have documentation and annual clearance by neurologist


Brain tumors

  • Must be at least 1 year from surgical treatment and have no neurologic deficits. –
  • Must present results of imaging that report that driver is tumor-free and clearance from neurologist and physiatrist.


Multiple Sclerosis

  • Annual MRIs required and the driver will be disqualified if the MRI changes significantly over the past yea
  • Must present a report with clearance from neurologist annually.
  • The driver may also be disqualified if high risk for infection due to medications that suppress the immune system


Peripheral Neuropathies

  • Must present written report from neurologist with clearance for driving; must pass pain and vibratory sensation testing during the exam


Tramatic Brain Injury

  • Must present written report from neurologist with date of onset, severity (mild/moderate/severe) and any associated seizuresDisqualifying Conditions:

Myotonia, Stiff Man’s Syndrome, Polymyositis, Meniere’s Disease, Metabolic Muscle Diseases, Systemic Peripheral Neuropathy, Neuromuscular Junction Disease (Myostenia Gravis), Muscular Dystrophy, Nonfunctioning Labyrinth, Labyrinthine Fistula, Construction Apraxia, Alzheimers Disease or other Dementias, Frontal Lobe Disorders, Vertigo (unless benign positional)
